I have made it easy for you to request an
appointment on-line.
Just fill out the simple form below, and I will
receive your request via email. Be sure to
include a contact number so that I can call you to
arrange the details of your appointment or to
answer any questions you may have.
*When requesting an appointment, please keep in
mind that if you are trying to schedule a "same
day appointment" you will need to call instead of
filling out this form.
New Clients:
First appointments are made over the phone only.
Once we have established a date/time for your
first session, please complete and print out an
intake form and
bring it with you to the session. This can be
found by clicking on the FORMS tab. If you prefer,
you can fill out a form when you arrive for your
session but having it ready saves time.
